- I hereby irrevocably agree and consent that Dream Masters Studios, LLC. (herein referred
to as
- “DMS”), a Missouri limited liability company, whose complete address and
contact information is
- stated in the top bar of this page, and its assigns, may use all or part, or none of the
video taped,
- still photographed, or sound recorded footage, or ancillary footage recorded of me,
staged or
- spontaneous, for archival footage that may be used in the production of any and all
types of
- informational/educational/entertainment programming in which this footage would enhance,
and/or
- round out the particular program(s) in which this footage may ultimately be used.

- DMS producers have the right to use my picture, silhouette and other reproductions of my
likeness
- and voice in connection with any motion picture or television program in which this
footage might
- be incorporated and in any and all advertising material promoting it.

- DMS producers have the right to edit my appearance as they see fit for the overall
benefit of the
- program(s) in which it is used and the promotion of such program(s).

- DMS shall have all rights, titles and interests in any and all results and proceeds from
said use or
- appearance.

- The rights granted DMS producers are perpetual and include the use of this footage in
any medium;
- all or in part of the program may be shown, including motion picture exhibition,
broadcast and
- cable television, internet entertainment vehicles, video cassette, DVD, CD’s, or
any other mediums
- currently unknown or yet to be developed.

- This consent is given as an inducement for the DMS producer to work with me on the
project titled:

- I understand that Producers will incur substantial expense in reliance thereof.
- DMS producers are not obligated to make any use of this interview or exercise any of the
rights granted
- producers by this release.

program.
